Job Description
You will be:
Joining the Hospital Discharge Team; your role will be to:
- Hold a case loads of service users to facilitate safe and timely discharges, by putting in place the appropriate care plans and packages.
- Collaborating with other healthcare professionals to ensure comprehensive care.
- Addressing any urgent or complex situations.
- Maintaining an accurate records of patient interactions.
- Registered to Social Work England and has completed an ASYE.
- The ideal Candidate will have direct experience of working within a hospital setting within Adult Social Work.
- Right to work in the UK.
- A Dedicated Consultant specialising in their Sector to ensure candidate satisfaction and matching you to the right role.
- A smooth compliance and payroll system.
