Pertemps are recruiting for a qualified and expeirneced Adult Social Worker to join a busy Acute Hospital in Boston, Lincolnshire. The postholder will be completing proportionate assessments to support a person's discharge from hospital in a timely manner. Utilising Home first principles and strengths-based practice the worker would be supporting the individual, working with their families and carers, and liaising with multi-disciplinary teams to support discharge to the person's own home where possible.

Being able to work collaboratively with Health and the Voluntary sector is a must. Knowledge of the Care Act, Mental Capacity Act, Continuing Healthcare and Safeguarding practice are essential to the role as workers for these positions will support junior members of the team.

This is a 7 day service - working as part of a rota at weekends is included in the job role.
