Experienced Agency Social Worker, Hospital Discharge Team
Pertemps are recruiting for qualified and experienced Social Workers to join the Triage and Response Team, integrated within the discharge teams at Southmead and Bristol Royal Infirmary Hospitals, and employed by Bristol City Council.
The postholder will have a passion for enabling service users to leave the hospital in a timely manner with appropriate support in place. Social Workers will work in coordination with health colleagues in discharge teams to provide appropriate, strengths-based, and timely support.

Responsibilities include completing Care Act assessments, mental capacity assessments, and other assessments in line with legislation, developing comprehensive discharge plans, and collaborating with medical staff to ensure seamless patient care transitions.

There is a requirement for Social Workers to be in the office everyday (unless there are reasonable adjustments to be considered).
