Social Worker, up to £38ph, Hospital Team, West Sussex
Pertemps are recruiting for a qualified and experienced Social Worker to join the Hospital team based in West Sussex. This role focuses on conducting hospital and care home reviews to assess and support individuals across the North of the County.
 
Key Responsibilities:

• Conduct comprehensive Care Act and Mental Capacity assessments for individuals in hospital or care home settings.
• Evaluate care plans to ensure services align with individual needs and improve outcomes.
• Liaise with multidisciplinary teams to facilitate seamless hospital discharges and appropriate care placements.
• Review existing care arrangements, ensuring compliance with health and social care policies.
• Work collaboratively with healthcare providers, families, and carers to achieve person-centred outcomes.
• Record accurate case notes and reports in line with organisational standards and statutory guidelines.

 
Requirements:

• Qualified social worker with current registration with Social Work England.
• Minimum of 2 years’ post-qualification experience in a relevant setting (hospital discharge planning or care home reviews)
• Strong understanding of the Care Act and Mental Capacity Act.
• Experience conducting assessments and reviews in health and social care environments.
• Excellent communication skills to engage with diverse stakeholders, including residents, families, and professionals.
• Ability to work independently in a fast-paced environment while managing competing priorities.
