Experienced Social Worker - Children's Transitions Team
We are seeking an experienced social worker to join our dedicated Children's Transitions Team. This team provides essential statutory social work services to our most vulnerable disabled young people across the Cheshire West borough. Our goal is to prepare these young individuals for adulthood and ensure a smooth transition into adult social care.
 
Key Responsibilities:

• Deliver statutory social work services to disabled young people.
• Support young people in their transition to adulthood and adult social care.
• Work collaboratively with families, carers, and other professionals.
• Assess and address the needs of vulnerable young people.
• Ensure compliance with relevant legislation and policies.

 
Requirements:

• Proven experience in social work, particularly with disabled young people.
• Strong understanding of transition processes and adult social care.
• Excellent communication and interpersonal skills.
• Ability to work effectively within a team and with external partners.
• Commitment to improving outcomes for vulnerable young people.

 
 
Requirements:

• Hold a Social Work England registration 
• Eligibility to work in the UK
• Full UK Driver’s Licence
• Post-qualifying experience 
• Degree/equivalent in Social Work
