We are seeking an experienced social worker to join our Park Lodge Community Mental Health Team. This role involves contributing to the effective delivery of Adult Social Care Services within an integrated Community Mental Health Team, focusing on safeguarding, support planning, and promoting the wellbeing of vulnerable adults and older people.
 
Key Responsibilities:

• Conduct assessments to identify individual needs and co-design support plans.
• Ensure regular monitoring and reviews of care plans.
• Manage your own caseload while maintaining the reputation of the Department and Council.
• Comply with national and service procedures and guidance.
• Apply the Mental Health Act, Mental Capacity Act, and Care Act in your practice.
• Facilitate hospital discharges from mental health wards.
• Attend tribunals and complete social circumstance reports.
• Assess and source support based on individual needs.

 
Requirements:

• Proven experience in social work, particularly within mental health services.
• Strong knowledge of the Mental Health Act, Mental Capacity Act, and Care Act.
• Experience with hospital discharges from mental health wards.
• Experience attending tribunals and completing social circumstance reports.
• Ability to assess and source support based on individual needs.

 
 
 
Requirements:

• Hold a Social Work England registration 
• Eligibility to work in the UK
• Full UK Driver’s Licence
• Post-qualifying experience 
• Degree/equivalent in Social Work
