Contract role – Hospital Discharge Team

Pertemps are recruiting for experienced Adult’s Social Workers in the Merseyside/Cheshire area
This is to work in a Hospital Discharge Team.
Key responsibilities
- Risk assessment and safeguarding management. Being able to respond appropriately.
- MDT working and being able to respond appropriately.
- Strength Based Assessment
- Person Centred Care
- Advocacy and Empowerment
- Mental Health Act
- Care Act 2014
