Social Care Assistant, Central Assessment Review Team, Redbridge

Social Care Assistant in the Central Assessment Review Team (CART) for a 6-month fixed term post (with potential to become permanent). 
About Community Health and Social Care services in Redbridge, London.  
  
The Social Care Assistant post sits within the Central Assessment Review Team (CART) team whose role is to identify and achieve safe savings. The post holder will be involved in carrying out Care Act assessments of care packages. The SCA will report directly to the Senior Social Worker and Head of Service. The CART team works in conjunction with the Community Health and Social Care teams and consist of district nurses, social workers, rehabilitation therapists, occupational therapists, MH workers, LD workers. We work in partnership with other health and social care teams such as specialist nursing or children services. This integrated way of working allows us to deliver high-quality person-centred services that meet the needs of the residents. Our services are innovative and rewarding for both clients and the workers. 
  
Our Central Assessment Review Team is looking for an enthusiastic individual to help us to deliver:  
  

• A strength-based approach using People Matter model through the 3-conversation model. 

• Person-centred holistic support planning to support people to maintain their independence by promoting health and wellbeing. 

• A joint assessment approach, which covers both health and social care needs, and a single point of contact for people and their carers. 

• Assessments for minor pieces of equipment and adaptations which will support the person to remain independent.
