Transitions Social Worker, Wolverhampton  
 
- Adult social worker within the SEND Family Help Service supporting young adults from 17.5 – 25 whilst they transition from children’s into adult services. 
- The role consists of working with a variety of agencies including health and education to identify plans that enable adults to access their communities. 
- Would suit someone who is experienced in Mental Capacity and Care Act assessments. 
- DoLs Trained ideal but full training given 
- Full time role with flexible working 
- 2 office days per month 
- Monthly supervision 
- Full training given
