An exciting opportunity has arisen for a Social Care Assessor to join the Preparing for Adulthood Service.
We are an established and diverse team of professionals which has a strong focus on development and progression for those for whom this is a priority. You will have an opportunity to truly make a difference to the lives of young people approaching adult social care for the first time or have already been receiving support from the service (and their carers).
The office is based conveniently in Wandsworth Town Centre with a 10-minute walk from Wandsworth Town Station.
The team’s focus is to provide a holistic, personalised, strength-based service to residents between the ages of 18 and 25 who have physical, learning and sensory disabilities as well as mental health social care needs.
We are looking for someone who shares our values and commitment to improve the lives of our residents with a keen interest to learn and innovate and a commitment to strengths-based practice and prevention.
In this role you will:
• assess and support plan applying the principle of good conversation, strengths-based practice, motivational interviewing (both in the community and in a person’s home)
• assess for eligible care needs in accordance with the Care Act (2014) and input urgent care packages if required
• signposting people to other services that will be beneficial for them
• completing review preventative support that have been put in pace
• have a strong working knowledge of preventative services, enablement and link people in with these promoting a ‘try it and see’ ethos
• have an understanding the Care Act 2014 and the roles and responsibilities of a Social Services Department including safeguarding
This is a busy team with a fast throughput of work, therefore excellent time management skills and the ability to manage your own caseload is essential. We would like to hear from people who are passionate about delivering the best services to our service users.
Essential Qualifications, Skills and Experience: A social care qualification (e.g. NVQ II, III, etc.).
