We are recruiting for motivated qualified social workers to join our busy team. This role is with one of the UK’s highest performing local authorities, serving as one of the best places to live and work in.
 
This role is based in the Transitions Service; you must be SWE registered and have experience in a similar team.
 
A social worker is required to join a newly formed Transition Service to help clear a backlog of care act assessments. It is preferred you have experience of completing strength based assessments and have some knowledge of adult services including how children transition across the two services.
 
Hybrid working is available – you will require your own transportation to complete visits to young people in their placements or at home.
 
You will be involved in multi-agency meetings & planning sessions with the young person and their parent carer to produce the transition plan for the young person.
