Key Responsibilities:
• Assist the Team Manager in the day-to-day operations of the CMHT.
• Conduct high-quality social care assessments in line with the Care Act for residents referred to the team.
• Deliver timely and effective decision-making to meet service user needs.
• Provide expert guidance to staff on problem-solving, case management, and team processes.
• Promote collaboration and strengths-based practices within the multi-disciplinary team (MDT).
• Ensure adherence to statutory frameworks, including the Mental Health Act.
