Key Responsibilities:
• Undertake assessments and reviews, and commission services in line with departmental policies, procedures, and statutory guidance, such as The Care Act (2014).
• Actively participate in safeguarding adults’ procedures, including investigations and serving as a Safeguarding Lead where necessary.
• Collaborate with other teams, providers, carers, and external agencies to deliver outcome-focused services to individuals and families.
• Adhere to departmental policies, Social Work England standards, and the Health and Care Professionals Council’s Code of Conduct.
• Empower individuals to make informed decisions, in compliance with the Care Act (2014).
