Adults Social Worker, Leeds
Hospital Team

We are looking for a social worker with experience of working in Adult Social Care, hospital discharge team. The social worker should have a good understanding of the social worker's processes including assessment, intervention and review and the key legislation that underpin this process. They should have the ability to undertake assessment of service users and carer needs using a person-centred approach in accordance with agreed statutory and directorate procedures. Able to assess capacity and coordinate best interest meetings and decision making for those who lack capacity to make decision for their discharge destination. Able to assess and manage risks and ensure any safeguarding is dealt with in timely manner. To have a good understanding of the strengths-based approach and how to apply this in practice.
