Contract role
Vitalis are recruiting for an experienced Social Worker to work in a hospital team in Westminster Council.
Key Responsibilities:
- Triaging referrals on behalf of the Tri-Borough Hospital Discharge Team
- Completing quick assessments for residents of the Tri-Borough
- Undertaking Care Act Assessments and developing Support Plans
- Completing Safeguarding Concern Assessments (Section 42.1)
- Setting up care packages to facilitate safe discharge
- Providing information and signposting to A&E staff
Essential Requirements:
- Social Work England Registration
- Proven experience in hospital discharge work
- Strong knowledge of the Care Act and safeguarding procedures
- Ability to work in a fast-paced environment and manage competing priorities
