Qualified Senior Social Worker – Integrated Discharge Team – Bolton
Paying £30 per hour 

The Talent Crowd are recruiting for an Experienced Social Worker to join an Integrated Discharge Team in Bolton.
Requirements:
- MDT working
- Be able to complete Care Act 2014 Assessments
- Have confidence in completing Mental Capacity and Best Interest assessments
- Be able to manage and respond in a fast paced environment
- Prioritise case load and be adaptable to re-prioritise as service allows
- Have awareness of Discharge pathways and models
- Knowledge of Liquid Logic system would be desirable
- Complete duty function as part of a rota
