Pay rate up to £24.27 per hour
The Talent Crowd are recruiting for a Social Care Assesor to join an Adult Social Care Team in Liverpool.
The Agency Social Care Assessor will be working within Adult Access Team which is located at Cunard Building, proving secondment cover to the Team. Some of their duties will involve providing assessment and reablement support , as well as assessing eligibility for ongoing support within the community. The duty to prevent, reduce and delay is alongside care and support provision and applies to all interventions. The team works with adults over the age of 18 and have a varied caseload including include hoarding concerns, substance misuse, older people, mental health, learning disability amongst others. Liquid Logic is the programme used and any worker will need to be able to utilise this proficiently.
