Job Description

Occupational Therapist
Location: Wolverhampton

Job Purpose:
 
  1. To take a lead responsibility for some complex assessments and undertake other complex assessments jointly with Community Care Assessors or Social Workers.
 
  1. To offer specialist advice to the team on clients’ functional, psychological and physical abilities using own professional knowledge base.
 
  1. To take responsibility to ensure that appropriate parts of service packages (eg equipment and adaptations) are processed and monitored, in liaison with other departments.
 
  1. To provide leadership for junior staff, through supervision and appraisal.
 
  1. To undertake assessments and write reablement plans, monitor outcomes and provide professional support to reablement workers

Principle/Main Responsibilities & Tasks:
 
Clinical:
 
  1. To be responsible for allocating O.T. referrals that are made by the area team and give advice to the team regarding matters related to Occupational therapy.
 
  1. To provide specialist assessments and programmes of care in liaison with multi-disciplinary teams.
 
  1. To provide specialist moving and handling assessments and make provision for training and equipment, as appropriate.
 
  1. To ensure that any necessary assessments and provision of equipment and adaptations are carried out, in accordance with the local authority policy and advise the Head O.T. of any inability to fulfil commitments.
 
  1. To provide assessments and treatment of a rehabilitative nature, where appropriate.
 
  1. To provide O.T. advice and support to other agencies, relatives and informal carers.
 
  1. To apply a high level of understanding of the effect of disability and provide training and advice on lifestyle changes and adaptations and optimise functional ability and the ability to live in the community.
 
  1. To carry out specialist assessments for the team to facilitate appropriate placements.
 
  1. To assist with resettlement into the community from residential homes and out of area hospitals.
