£38.15 per hour

About the role:
- To undertake scheduled reviews and monitoring visits to residential and nursing homes.
- To promote and enhance the lives of people living with dementia within a residential/nursing placement.
- To work with providers to improve the opportunities for people living with dementia within a placement setting.
- To provide reports to the Team Manager on visits to placement settings.
- To liaise with other professionals and out health colleagues.
What you need:
- A degree in Social Work or equivalent
- Registered with Social Care Wales
- at least 3 years post qualifying experience as a social worker & not left a LA permanent contract in Wales within the last 6 monts
- Completed relevant post-qualifying training
- Enhanced DBS check
- Excellent communication and written skills
