£35.00 per hour
Scunthorpe
37 hours p/w, 7 day rota
About the role
- The Home First Hospital Team have 1 x Full Time vacancy available
- The position is working over 7 days, on a shift pattern within the Integrated Discharge Team (IDT) in Scunthorpe General Hospital.
- Hospital Based and the role is not suitable for home working
- To complete information gathering on the wards at Scunthorpe General Hospital. Hold MDT conversations with professionals to decide the pathway for a safe and timely discharge.
- Be able to respond to A & E and plan for care from hospital.
What you need:
- A degree in Social Work or equivalent
- Registered with Social Care Wales/ Social Work England
- Completed relevant post-qualifying training
- Enhanced DBS check
- Excellent communication and written skills
