About the role:

• To undertake the role of personal advisor and to work with the social worker and other colleagues to the benefit of the young person 
• To provide support to a range of young people from 16+ who require advice, support and assistance in preparing for independence and who are preparing to leave care. 
• To ensure that young people aged 16+, are living in suitable accommodation, have the opportunity of being engaged in work, training or education and that they are not socially isolated.
