About the Role As an adults Social Worker, you will play a vital role in supporting adults and their individual needs and create personalised support plans, ensuring they receive the necessary support and services to achieve positive outcomes. Your responsibilities will include:

• Conducting assessments to identify individual needs and create personalised support plans.
• Working collaboratively with multi-agency teams to deliver coordinated care.
• Supporting individuals to access resources and services that promote independence and improve quality of life.
• Safeguarding vulnerable adults by identifying and responding to risks appropriately.
• Maintaining accurate and timely case records in line with statutory requirements.

About You
You are an experienced social worker with strong interpersonal and problem-solving skills. Your ability to build trusting relationships and navigate complex cases with empathy and professionalism sets you apart. You will have:

• A degree in Social Work and registration with Social Work England.
• Proven experience in working within adults services.
• A sound understanding of relevant legislation, policies, and procedures.
• Excellent communication and organizational skills.
• A commitment to promoting equality, diversity, and inclusion.
