As a Pathways Advisor, you will play a key role in supporting young people aged 18-25, helping them navigate the challenges of life after care and ensuring they have the tools and support to reach their full potential. This is a rewarding role that offers you the chance to make a lasting impact in a young person's life.
In this Role you will
- Provide direct support to care leavers, guiding them through the transition from care into adulthood.
• Work closely with the young person’s network, including families, friends, and partner agencies, to deliver child-centred systemic practice.
• Lead and facilitate group and enrichment activities to support young people in achieving their goals and aspirations.
• Oversee the creation and delivery of high-quality Pathway Plans to help young people meet their personal goals.
• Build strong working relationships with statutory and voluntary organisations, ensuring collaborative and holistic support for each young person.
What We’re Looking For:
• A Level 3 qualification or higher.
• Demonstrated experience working in a similar role, ideally with care leavers or young people in need of support.
• A commitment to providing compassionate, child-centred support to young people.
• Ability to work independently and as part of a team.
• Access to your own vehicle with relevant business insurance to support your role.
