Social Worker - Single Point of Access
£35.00 per hour
North Lincolnshire
37 hours p/w, min 3 months contract

About the Role:
These roles will include some weekend working as part of working as part of the hospital discharge team.  
Working as part of the Single Point of Access Team which will include  home visits and completing Care Act Assessments, working within the hospital integrated discharge team based at Scunthorpe General Hospital. 
Completion of MCA/BI as necessary 

What you need:

• A degree in Social Work or equivalent
• Registered with Social Care Wales/ Social Work England
• Completed relevant post-qualifying training
• Enhanced DBS check
• Excellent communication and written skills
