Are you passionate about supporting young adults with complex needs as they move toward independence?
We’re looking for an experienced Qualified Social Worker to join a dedicated Preparing for Adulthood Team in North Manchester. This vital role focuses on supporting young people aged 16–25 as they transition from adolescence into adult life — helping them navigate services, build confidence, and live as independently as possible.
You’ll play a key role in safeguarding, Care Act assessments, and multi-agency planning, supporting individuals with learning disabilities, physical health conditions, autism, and mental health needs. 🔍 Key Responsibilities
- Manage a caseload of young adults (16–25) with a range of complex needs
- Carry out Care Act 2014 assessments, support planning, and reviews
- Work closely with families, carers, and professionals to coordinate transitions
- Support individuals to develop life skills, access education, employment, housing, and health services
- Screen and triage safeguarding concerns and contribute to protection planning
- Apply legislation including the Care Act, Mental Capacity Act, and Mental Health Act to inform practice
- Contribute to duty work, decision-making panels, and multidisciplinary planning meetings
👤 Who We’re Looking For
You will be:
- A Qualified Social Worker registered with Social Work England
- Experienced (min. 3 years post-qualified) in Adult Social Care, Transitions, or Learning Disabilities
- Knowledgeable of relevant legislation: Care Act 2014, Mental Capacity Act 2005, Human Rights Act, Mental Health Act
- Skilled in person-centred planning, risk management, and multi-agency working
- Empathetic, collaborative, and confident in advocating for service users’ rights
- Experienced in working with young people moving into adult services
