Job Description:
- To provide an initial assessment of service users with complex care and therapy needs across the teams within the Access and Independence Service: First Response, Reablement and the Assessment Service.
- Supporting Service Users in their environment ensuring right support and a plan is provided.
- Assessment and provision of equipment to improve independence.
- Managing a case load.
- Completing necessary assessments.
- Previous experience as an Occupational Therapist.
- Right to Work in the UK, this role does not provide sponsorship.
- Diploma / Degree in Physiotherapy / Occupational Therapy.
- HCPC Registration.
