Job Description

You will be:
Joining the Adult Integrated Hospital Discharge Team; your role will be to:
- Cover intermediate care bed-based discharge work.
- Work within a multidisciplinary team to ensure residents receive a safe and timely discharge.
- Completing assessments and ensuring a care plan is in place.
- A manageable caseload.
- Work for a ‘Good’ Ofsted rated Council.
- Supportive management Team.
- Training and working equipment provided.
- Flexible working arrangements.
- A Social Worker Bachelor’s Degree and an ASYE completed.
- The ideal Candidate will have direct experience of working within Adult Social Work, it would be beneficial if you have previous experience of working within an Adult Hospital Discharge Team.
- Registered to Social Work England.
- Right to work in the UK.
- A Dedicated Consultant specialising in their Sector to ensure candidate satisfaction and matching you to the right role.
