Job Description

You will be:
Joining the Adult Transitions Team; your role will be to:
- Supporting Young People aged 18 – 25 with a learning disability, supporting their needs under the Care Act.
- Completing visits and assessments.
- Building relationships with Young People and Families.
- A manageable caseload.
- Supportive management team.
- Training and working equipment provided.
- Flexible working arrangements.
- A reliable on rolling contract and a stable management team.
- Very competitive pay rate of £32.00 per hour.
- A Social Worker Bachelor’s Degree and an ASYE completed, as well as 2 years of Post Qualifying Experience.
- The ideal Candidate will have direct experience of working within Adult Social Work, it would be beneficial if you have previous experience of working within a Transition Team/ Learning Disability.
- Registered to Social Work England.
- Right to work in the UK.
